### Cause

Most general issues come from a plugin or theme conflict, a stale cache showing old results, or a setting that is not configured for your use case.

### Fix steps

1. **Check for plugin conflicts.** Temporarily deactivate your other plugins, then confirm the issue is gone. Reactivate them one at a time to pinpoint the conflict.
2. **Reinstall the plugin.** A clean reinstall clears minor glitches. Deactivate and delete Rename Media Files, then install it again and re-check your settings.
3. **Clear your caches.** Clear the browser cache and any WordPress or CDN cache so you are seeing the latest changes rather than a stored copy.
